Is the registered agent consent form required to be filed with the Louisiana Secretary of State?
Louisiana does not publish one separate statewide registered-agent consent form for all business entities. Instead, the acceptance is built into the operative filing for most entity types.

For domestic LLCs, Notice of Change of Registered Office and/or Change of Registered Agent (Form 983) states that a registered agent may change the agent’s own address by signing a statement containing the company name and the new address, with written notice to the company. For foreign corporations and foreign LLCs, Statement of Change of Registered Office, Registered Agent and/or Principal Business Establishment in Louisiana (Form 346) allows the registered agent to execute the filing when the only change is the registered-office address.
